A leading multi-speciality hospital in Kerala sought to address delays in inpatient discharge, which affected patient convenience, billing coordination, housekeeping schedules and bed availability.

The discharge process involved multiple interdependent stages—from clinical confirmation and summary preparation to nursing communication, charge closure, billing, payment or insurance processing and room vacation. Management required an evidence-based approach to identify the actual sources of delay and establish clearer accountability across departments.

What we did

  • Mapped the complete discharge journey from probable-discharge planning to patient exit and bed readiness.

  • Analysed more than 5,000 discharge-related timestamps and operational data points to identify delays, sequencing issues and process exceptions.

  • Reviewed discharge-summary completion patterns by department, time of day and stage of processing.

  • Distinguished clinical, nursing, billing, insurance and system-related delays instead of treating discharge delay as a single departmental issue.

  • Introduced previous-day probable-discharge planning, morning confirmation and time-bound monitoring of pending cases.

  • Redefined discharge SOPs, departmental responsibilities, escalation protocols and expected turnaround times for each stage.

  • Designed a case-level discharge tracker and management dashboard covering summary completion, nursing intimation, billing, payment, room vacation and bed readiness.
